Beta 1.1 Server Errors

i want to start a thread here with all the new errors that servers come across and hopefully get answers from the ESF team on what to do to fix them, the first one i am submitting is ,

FATAL ERROR (shutting down): Bad entity in IndexOfEdict()
Sun May 25 00:24:54 PDT 2003: Server Died
