Best Opening: Rurouni Kenshin (song: Siam Shade - no Junjou na Kanjou)
Best Ending : Rurouni Kenshin (song: T.M. Revolution - Heart of Sword)

Anything and everything Inu-Yasha. For such a badass Anime that actually turned into an repetitively bad Anime after the first 50 eps--there wasn't a single opening or ending that wasn't great. They are all memorable; I couldn't possibly list a single opening or ending and actually be sure about it. So I'll go with two different Anime's on the matter.

Best Opening -- Dan Dan Kokoro Hikareteku -- Dragon Ball GT
Best Ending -- Ryuusei -- Naruto

I don't know, I guess one of the endings of Inuyasha. I think it's the one that showed a lot of Sesshomaru.

I still think best OP/ED is in Elfen lied, the sadness of the music just fits in the series better than anything.
Otherwise FullMetal Alchemist has some of the best OP/EDs around
