Unless it's GT/AF.
In GT Goku even grows up when he goes SSJ4, loses the top of his suit and there are changes about his shoes AFAIK. He becomes little again after descending and gets back the top of his suit.

Its a pic of SSJ3 with inverted hair colours. And wrong number 3 is Gogeta taken directly from GT and recolored his hair sylver.
As for the wrong number 4. That one is based on a sketch made by toriyama himself for SSJ4. He never made anything more than the sketch. It was a furry SSJ3 with a tail ^^
So thats about as official as you can get and even that was probably just a W/E moment in his life ^^
