The missile shield, as it exists today, is and has always been a farce dedicated to making Americans feel safe in the face of certain destruction. We're supposed to be overhauling it fairly soon to take advantage of our breakthroughs in laser technology (both guidance and offensive capabilities; think star wars). We're also taking the space-based route, where we'll basically fire tungsten rods at ballistic missiles once they enter sub-orbital space.

All in all, I'm not worried. The one nice thing about a military industrial complex is we get some nice toys.

It's also important to note that their entire defense system revolves around using nukes. What happens if we shoot each of those ICBMs from space seconds after they liftoff? What's their back up plan? Captain Iran was right when he said nukes were a relic of the 20th century. They're great for looking tough, but, unless your goal is to destroy humanity, they're utterly useless. You need more than just big nukes.

You don't have to attach nukes to a missile. Smugle the nukes into a city and remote detonate them.

1) You're pretending ICBMs aren't the topic of discussion
2) You're underestimating how difficult it would be to transport a nuke of any size or magnitude.
3) You're assuming none of the intelligence agencies of the world would know what was being planned.
4) The guy dragging around a full blown nuke would probably die of radiation poisoning before he ever detonated it. If it's a dirty bomb, it isn't going to be powerful enough to do anything other than really, really piss off the host nation. If the intelligence agencies of the world didn't know before the detonation of the bomb, they'll certainly know who was responsible afterward.

The missile defense shield as built was never intended to stop a country like Russia or even China from all out nuclear war. That's been stated several times at all levels of the project. It's meant from stopping smaller nations like Iran and North Korea from launching and succeeding in hitting targets.

I'm of the opinion that it's a cover to develop tactical laser weaponry anyway, which they pretty much have done. I've heard some interesting applications as a weapon for the ABML, and they've already tried it as a gunship weapon. As if AC-130's weren't scary enough.
